Retirement Home
Retirement Home residents rent their rooms on a month to month basis.
Individual residents who rent two rooms will receive a discount.
Semi Private Rooms may be required for SSI residents and rates my be available upon request.
Levels of Care:
Under the "levels of care" model each month the additional care that a resident receives will be
entered into a spreadsheet which will generate a total score.Residents will then be
placed into one of six "levels of care" based on their total score. Each level of care is assigned
a daily rate, which will be billed to the resident. A copy of the "Levels of Care" will be available
upon request.

Level 0 (0 Points) $80.00
Level I (1-15 Points) $83.00
Level II (16-30Points) $86.00
Level III (31-50 Points) $90.00
Level IV (51-70 Points) $95.00
Level V (71-100 Points) $100.00
Level VI (101-130 Points) $106.00
Services Included:
Meals - Three meals per day and snacks
Utilities (except phone)
Housekeeping - Once per week
Linens & Laundry - Bathroom and Bed linens
Medication Passes
Minor Dressing and Personal Assistance
24-hour supportive care
Room Changes - Residents who wish to change rooms will be allowed to do so
on a space available basis. A charge will be applied to defray
the cost of the room change....................$600.00 per change

Carport or Garage Parking....................$5.00 per month
In Room Meal Tray.................................$2.00 per delivery
Transportation........................................$ .55 per mile
